For Your Appointment

Appointments

Please call the office to initiate the appointment process. If you are unable to keep your scheduled appointment, please call us at least 24 hours prior to your scheduled appointment or message us via the portal so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery. You will receive an invitation to register on our Athena Portal to fill out paperwork prior to your scheduled appointment.

Fees & Payments

We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, Visa, American Express, and Discover for your convenience. Co-pays are required at the time of service. Payment is the patient's responsibility.

Prescriptions & Renewals

Please request all prescriptions and authorizations during our regular office hours, when our full records are available. Prescription refills requested at other times will be filled only for extreme or emergent circumstances. Please allow us advance notice before you are out of your prescription. We require 24 hours on all prescription refills to be reviewed and signed by the provider.
